St. Anthony New Brighton Schools Health Services encourages periodic health exams since prevention and treatment of health conditions will enhance student’s attendance and performance in school. Consult your health care provider regarding specific recommendations.
Health & Physical Examination forms are distributed to sixth and ninth grade students, in the spring of the year. They can also be obtained through the Health Office or online at any time.
Athletics
Students participating in school athletics must follow the athletic registration process. An updated High School League Physical Examination form is required to be completed and submitted every three years. A health questionnaire shall be completed annually and could indicate the need for a physical examination prior to participation.
