Annual Health and Emergency Forms
In August, parents will receive a health and
emergency form. The front side will contain specific information about
your child which should be reviewed and updated, paying particular
attention to correct phone numbers and emergency contacts.
The back side should be completed by the parent, signed and dated.
This form should be returned to the school by the date indicated at the
bottom of side two. (Dates will change each year based on the school
calendar and may be different for the middle school and high school.)
Our goal is to have all forms returned by the first day of school each
year.
Data on this form will be used to keep parent and emergency contacts
as well as medical status information up to date. This information is
especially critical should there by any medical emergency involving
your child. It is important to notify the school of any changes during
the school year (phone number, address, etc.)
